This form needs to be DOWNLOADED to a computer to be filled out digitally. It does not work fully if filled out in a web browser. It IS possible to print from the web browser and fill out manually- this can be scanned and emailed to us. For those trying to use a cell phone, you will have to create a text box and manually enter the data into each field if you do not have Adobe PDF on your phone. Again, it must be downloaded to the device. |
Reminders for Entering a USDF Recognized Competition
A complete entry consists of:
TWO entry forms and sets of waivers- this is licensed as two separate competitions which allows you to qualify for championships once per day
Signed signature page and waiver for each entry due to different licenses and dates. These can be done in person when you go to get your number from the show office if you have any issues with the fillable PDF or do not have adobe docusign.
Vaccination record reflecting a flu-rhino shot within 6 months
Current negative coggins (within 12 months)
A complete set of cards from USEF Affiliates Membership Verification Service (eqverification.org)- select UDSF as your affiliate membership, you need cards for rider, owner, horse
Make sure you have also completed safe sport as not meeting that requirement will delay ride times and stabling chart for everyone
